Sev,
I am pretty sure that given Mayn's medical condition, he probably was not taking to many if any meds for the arthritis. I know he took other stuff but I never heard mention of pain killers. Besides, those stronger med for that stuff are murder on your liver and kidneys so I think he might not have been able to take the stuff. Manthey would know for sure because he was taking care of Mayn.
Maynard's boyish attitude is one of the things that made the gig so great. No matter how !#$%^#$%&^ you day was, you could always count on him being in the best disposition and just so eager to get out there and put on a good show for the people. If you let it, it was highly contagious and I wanted to be on that trip so bad that I let him take me away every time.

IamBrass...
Coming from the bottom... let me think now... I am pretty sure I have seen quite a few of the top dogs play that way actually. I think anyone that is a down-streamer is going to have that approach but please don't quote me on that as it is merely an observation. For me it's different because of my horn angle and jaw movement (and MP placement too). I think I would cut my lower lip off if I aproached the horn in that manner. Now when I use the other grip (the new, more "traditional" one) I don't know if I pressing from bottom to top but I can feel a distinctly opposite sensation from my oft used grip. Jaw allignment is different, lower lip roles in not out, lower jaw stays in place (actually moves up so as to close my teeth), apperture tightens as opposed to dialates. Chops seem to "frown" as oposed to the Maggio "pucker" that I am accustomed to. I mean even the way I breath is different and that has been by far been the hardest habbit to break. The breathing for the newer grip is shallower and to the chest with the shoulder rising to my ears a la Waynard as I assend. For those of you who know me and my posts, that is the complete oposite of how I normally do things (I have always been a "pull your lungs down to you crotch" Maynard type breather). It's a whole other world than the way I have played my whole life I tell ya and luckily it is so different from my normal approach that the two never bother each other. Doubles are slightly more difficult and not as big with my "normal" grip but triples are easier than play a double A (that's my go to note in the upper register which seems to lock in no matter how much or little lip I have left).
And to answer you question, YES! I have way too much time on my hands HAHAHA.
